Pre Discharge Program
The Pre Discharge Program provides;
- Practical coping and life skills enablng a more seamless transition from Cefn Coed Hospital to day services.
- Taster courses within the hospital setting.
- Transport links to and from Cwmbwrla Day Centre, Orchard Centre and the Hospital.
- Access to these groups will give people the opportunity to be referred to the CRERATE partners and reduces the inactive period that could occur after discharge from hospital.

- Health and Fitness
- Tai Chi
- Cookery
- Computers
- Arts and Crafts
- Relaxation and Social Activities
- Gardening
- One to One Work
- And Lots More
Accss to these groups will provide opportunities for people to be referred to all CREATE partners.
