This leaflet aims to give you, your family and friends a brief outline and some information about the Swansea Crisis Resolution Home Treatment Team
The service is for adults of working age who have a severe mental illness and are experiencing an acute episode of this illness. The Crisis/Home Treatment Team offers alternatives and choices to all its clients where possible, to reduce the need for hospital admission or to reduce duration of stay for inpatients. Our team comprises a range of professionally, trained staff, and support workers who are fully integrated in mental health services and provide continuous care, offering a broad range of intensive support to its users.
Doctors, Nurses, Social Workers, Occupational Therapists and Support Workers and Administration Staff.
The team provides a 9am to 9pm response, 7 days a week, all year round, including Bank Holidays. The service is normally offered for up to 6 weeks.
We are respectful and sensitive to people's race, culture, gender, sexual orientation or disability; we acknowledge the uniqueness of the individual. We will attempt to create a partnership with our clients based on mutual respect, we see trust as a two way process between the client and team.
After an initial assessment an agreed action plan will allow the team to understand the client's needs in order to be able to offer advice to reduce and resolve distress. After further discussion and negotiation a care plan will be written by the care coordinator from the community mental health team in conjunction with CRHTT and service user/carers. Clients who have an existing care co-coordinator can expect them to remain involved in their care while supported by the Crisis/Home Treatment Team. Assistance will be given to the client to resolve any outstanding issues that may be causing concern; these will be dealt with on a step-by-step approach in accordance with their expressed wishes and needs.
In many instances people who are seen by a Home Treatment Team will request or require medication. After a full discussion with the client, any medication prescribed will be closely monitored by the team.
We endeavor to work closely with carers, family, friends, mental health services, social services, and voluntary organizations.
